2019 DPC Summit Scholarship Application

Eligibility and Guidelines
Scholarship Information
  • The number of scholarships varies depending on the funding level and the number of applicants.
  • Scholarship recipients are selected by a panel of judges from the DPC Summit planning committee.
  • The $500 scholarship is intended to help reduce out-of-pocket expenses (registration, travel, lodging, meals)
  • All scholarship checks are awarded at the event. You will need to register and handle all travel accommodations. If you are unable to attend the conference, your scholarship will be forfeited.
Eligibility Requirements
  • Membership Requirement: You must be a member of the American Academy of Family Physicians (AAFP), the American College of Osteopathic Family Physicians (ACOFP) or Family Medicine Education Consortium (FMEC) to apply.
  • Attendance Requirement: Selected Scholars will be required to attend the entire 2019 DPC Summit (defined as arriving no later than noon on Friday, June 28, and leaving no earlier than noon on Sunday, June 30).
Review Process
  • The applications will be reviewed and scored by the members of the DPC Summit Planning Committee. The applicants receiving the highest numeric scores will be chosen to receive the scholarship.
  • Every effort will be made to maintain a balance between the number of Family Medicine residents and the number of medical students
Dissemination of Funds
  • Checks will be distributed upon check-in at the registration booth at the DPC Summit. Each recipient must sign for the check, as well as complete a W-9 Form prior to receiving the check.
